HEROIN TRIALS: - Fostering Knowledge, but Also Misunderstanding (NEW: Aug 12'06) The trials of Switzerland, Netherlands, Germany etc. are to be applauded for demonstrating that heroin maintenance is feasible and effective for many. But the methodology also carries with it a downside: it purportedly compares heroin to methadone, even though eligibility requirements generally limit subjects to those who have already "failed" with methadone. An example: a German article (Die Tageszeitung, Junge Welt, 7 Aug) headlines: "Methadone or heroin?" and states the objective was to compare the two medications. The misunderstanding could probably have been avoided had the methadone non-responders been assigned to two arms using anything EXCEPT methadone - e.g., heroin and buprenorphine, or heroin and naltrexone, or . . . . Too bad; there’s enough confusion and misunderstanding in this field.
